Frequently Asked Question for COPD Program: 

  • Can I self-refer?

    Yes, we welcome self-referrals.

    COPD – those seeking COPD care, please note that a confirmation of diagnosis is required to be able to access all COPD program and services. If you do not have proof of diagnosis, a team member would be happy to assist you in acquiring it.

    Smoking Cessation – self-referrals are welcome for both group and individual care.

  • How can I get Nicotine Replacement Therapy (NRT)?

    Clients are requested to book an appointment to meet with the Registered Respiratory Therapist.   Upon assessment, they will advise you if you qualify for the NRT and how soon you can expect to begin NRT.  Please note NRT is not provided upon first visit.

  • Do I have to pay for Nicotine Replacement Therapy (NRT)?

    No, there is no cost for NRT at the VCHC.  The VCHC has partnered with the Centre for Addictions and Mental Health (CAMH) and their ministry funded smoking cessation program called STOP. The program provides clients with an available 26-week supply of NRT for those who qualify.

  • How do I get an assessment?

    We encourage you to contact a COPD Team member for assistance as every client has unique needs.  Assessments are based on reason for referral and/or goal.

Frequently Asked Questions for Diabetes Education: 

  • What is Diabetes?

    Type 2 diabetes is a chronic disease in which your pancreas does not produce enough insulin or your body does not properly use the insulin it makes. High blood sugars can lead to complications such as heart, kidney, eye disease and nerve damage.

  • How do I know if I am “at risk”of diabetes?

    The following conditions may contribute to the development of Type 2 Diabetes. However, having these risk factors does not necessarily mean you will develop diabetes.

    • Age 40 years and over

    • Family history of diabetes

    • Overweight

    • History of diabetes during pregnancy

    • Gave birth to a baby weighing greater than 4 kg (9lbs)

    • High cholesterol

    • High blood pressure

    • Polycystic Ovary Syndrome

    • Psychiatric disorders: schizophrenia, depression, bipolar disorder

    • Acanthosis Nigricans (darkened patches of skin)

  • What are the symptoms of diabetes?

    These are the classic symptoms of diabetes; it is important to note that many people who have type 2 diabetes may have no symptoms.

    • Frequent urination

    • Fatigue

    • Unusual thirst

    • and unexplained weight loss

  • What can I expect from my diabetes visit/appointment?

    During one-on-one appointments, you get individualized support. Our nurses and dietitians will help you to understand various aspects of your diabetes self-management. They will also support you to identify goals to help improve blood sugar control and quality of life.

  • What should I bring with to my diabetes visit/appointment?

    To make your appointment more meaningful, please bring following to your appointment:

    • Glucometer (blood sugar monitoring device)
    • Blood glucose (sugar) monitoring record
    • List of all your medications or medications in their original containers
    • Copy of your most recent blood work result from your doctor
    • Food record of what you usually eat
  • I have been living with diabetes for years. Why should I attend VCHC diabetes program/clinic?

    Meeting with a diabetes educator can inspire you to take the next step that you have been reaching for or help you to envision a “healthier you”.  At VCHC you will have access to a wide range of health care services and health professionals who may be able to assist you with issues related to diabetes such as foot care, financial issues related to diabetes care, healthy eating, trouble-shooting etc.

  • I have already attended a diabetes education program in the past, will I benefit by attending again at VCHC?

    Diabetes is a lifelong condition and its needs are evolving. At VCHC we have multiple services and support for you. Our clients can have access to other services such as exercise programs, foot care, social worker support etc., which will help you to better manage your diabetes care.

  • I have already attended an one-on-one appointment; Do I still need to attend a group session?

    Our group sessions cover different of topics related to diabetes care. This information forms a strong base for your diabetes knowledge.
