Allied Health – PACT

The Allied Health – People Accessing Care Teams (PACT)  program supports people with a variety of aspects of their health journey.

Who We Serve: 

Low- or moderate-income individuals experiencing one or more of the following:

Inadequate housing

Unemployment

Lack of transportation to appointments

Lack of extended health benefits

Lack of health coverage (no OHIP)

Unable to speak English or French

Have complex health issues

Supporting Solo-Practicing Primary Care Providers:

The team supports solo-practicing physicians and nurse practitioners in the community who do not have access to allied health services in-house. Services are offered in Western and Northern York Region.

Our Services Include: System Navigation Case Management, Mental Health Counselling, Nutrition Counselling, Physiotherapy, Chiropody, Respiratory Therapy (lung health), Type 2 Diabetes Education and Management. Click on each service below for more information
